Installation has made under the instruction. The site is made on Joomla 2.5. Error is not present. The script is not displayed on page. Here the address where the script is placed.

Hi, I have this script and so far it's been working great... Except I ran into a snag, I've added two fields to the sign up page and updated all other correlating pages as far as I can tell.
It validates all fields and seems to be working but it's not submitting to the DB but when I hit submit it just keeps sending an activation email for each click.
If you have an idea of what it could be a hint can work wonders.
